About Monarch's Residential Services at Putnam Pike

At Putnam Pike, Monarch provides supportive care in a residential setting for up to 9 individuals as they are discharged from hospital treatment. Putnam Pike’s living arrangements include:

  • Three suites, each one containing three bedrooms, one shared bathroom, an enclosed patio, den/common area and a kitchenette for light dining.
  • A large atrium/community room that will be set up for daily activities and special events for residents.
  • Prepared meals for breakfast, lunch and dinner.
  • A Safe Haven Room which is available to all residents where they can receive one-on-one support from staff during times of crisis.
  • 24/7 onsite care from dedicated health care professionals including Residential Managers, a Psychiatric Registered Nurse, Residential Treatment Staff, Certified Nursing Assistants and Certified Occupational Therapy Assistants.
  • Support for additional needs such as employment, physical health, finances and safety which may affect overall health, well-being and independence of residents

Jennifer Raphael-Guzman has nineteen years of experience in the nonprofit congregate care and mental health, providing oversight of the daily operations and  adhering to contractual and accrediting organization guidelines. She has worked diligently to network with state agencies, providers from other non-profits and internal agency colleagues. Jennifer is a passionate, committed individual who prides herself on seeing each obstacle as a learning experience and a chance to improve. She is inspired by and enjoys spending time with her her husband and three children. In her free time, she listens to music and attends the gym.
