Joel Lewis Finds Stability Through Tailored Care Management Services

When Joel Lewis connected with Monarch’s Tailored Care Management (TCM) program, he discovered the power of personalized support in helping him navigate the challenges of daily life with confidence and resilience. Living in Kernersville, North Carolina, Joel has managed to overcome obstacles in housing, work, and mental health with the guidance of his Care Manager, Beth Myers, and the dedicated TCM team at Monarch.

Tailored Care Management is a North Carolina Medicaid service designed to provide personalized support for individuals living with mental illness, substance use disorders, intellectual and developmental disabilities, and/or traumatic brain injuries. A team led by a Care Manager collaborates with each person to create a customized plan, connecting them with therapists, doctors, family, and community services to help them thrive.

Since joining the TCM program, Joel has received invaluable resources that support his independence. One of the most meaningful parts of his journey has been the certification of his dog, Maxine, as an emotional support animal. These skills have allowed him to contribute to his community in meaningful ways, including sharing home-cooked meals with his church family, where his baked spaghetti and barbecue chicken have become beloved staples.

“Joel has worked so hard to build his independence,” said Beth Myers, Joel’s Care Manager. “Through TCM, he’s gained valuable skills and has advocates to support him every step of the way.”

A part-time job with a Veteran’s Affairs contract company has also been a rewarding addition to Joel’s life, providing him with structure, a sense of purpose, and opportunities for positive interactions with veterans. The job, coupled with TCM’s continuous support, has been instrumental in helping him set and achieve goals that once felt out of reach.

As Joel continues on his path, he’s setting his sights on securing a more permanent housing solution. With Beth’s guidance, he’s exploring options that align with his financial goals and long-term vision for stability. Monarch’s TCM services have enabled him to develop a clear plan and connect with community resources, including legal assistance, that will support him as he moves forward.
