Primary Care Services

Monarch’s Primary Care Services support the whole health of established adult patients by offering select routine health care services alongside behavioral health care.

Primary care can help with everyday health needs, preventive care and ongoing support for common medical conditions. Services may include annual wellness visits, routine screenings, medication management, health education and care coordination with other providers.

Services May Include

• Preventive care and annual wellness visits
• Routine physical exams and screenings
• Chronic disease management for conditions such as diabetes, hypertension and asthma
• Sick visits, based on appointment availability
• Medication management
• Women’s and men’s health needs
• Care coordination and referrals
• Integrated behavioral health support
• Health education and lifestyle counseling
• Support for common health concerns, such as respiratory infections, urinary tract infections, mild rashes, minor sprains, medication refills and basic wound care

Emergency Care

Primary Care Services are not a substitute for emergency medical care.
