Eligibility

This service is available through the North Carolina Innovations Waiver, a Medicaid program that supports individuals with I/DD in community settings. Community Networking services eligibility is determined by an independent assessment conducted by NC Medicaid or its designee, and is provided according to an individualized service plan.
