Individual Support Services are provided to adults ages 21 and over with mental illness through (b)(3) Medicaid funding. The service helps individuals with severe and persistent mental illness live independently in the communities. It provides support with daily activities (e.g., preparing meals, managing money, cleaning the home) as he/she/they plan to move out into an independent living setting. This is a temporary service to help individuals as they move from a licensed facility or private home into their own home.
